If I want to write the following:

I am extremely allergic to nuts and almonds, including [a list]. If my food contains a single trace of that, I can get an anaphylactic shock and die.

would that look anything like this?

Sono estremamente allergica dei noci e delle mandorle, incluso/includando [the list]. Se ne si trova una singola traccia nel mio cibo (?), possa rendermi un shock anafilactico (??) e mi fa (faccia?) morire.

As you can see, I need a little help with the language Wink. Also, do you have any suggestions for opening phrases for this note? "Dear chef"? The thought is to give this note to waitors who in turn can give it to the chef, rather than explain everything to waitors who then forget half of it.

Sono estremamente allergica alle noci e alle mandorle, compreso (the list). Se ce ne anche una singola bricciola nel mio cibo potrebbe provocare uno shock anafilactico (not sure about this one)che potrebbe causare la mia morte.

Potrebbe! Condizionale! I knew that there was something wrong... I tried using the congiuntivo but that didn't sound right either. Thanks!

Laurie, rest assured. I'll bring all kinds of emergency medicines, but I'd prefer not needing them. I don't want to spend my few precious days in Italy in a hospital!

I suggest:
Sono fatalmente allergica alle noci ed alle mandorle, ed anche a (the list). Se la minima traccia di queste sostanze si trovasse nel mio cibo, provocherebbe uno shock anafilactico che potrebbe causare la mia morte. Smiler

i suggest you do not eat anything with chocolate unless you check the ingredients... you cannot for example with a CORNETTO AL CIOCCOLATO that would probably be with nutella and most chocolate have nuts derivates... also ORZATA that is a white liquid syrup used for fresh drinks is made from almonds and COLOMBA cake that u find in supermarkets have almonds. MARZAPANE and most sicilian cakes and sweets are made with almonds too.check all labels you can Wink
