I am retired with a Canadian pension and am moving to Italy. I understand that I will pay some income tax on my pension. Will I still have to also pay around 7% of my income to use the public health care system....or once I start paying Italian income tax, will my contribution for health insurance be decreased or eliminated?

You would need to pay to remain enrolled in the public health care system, regardless of paying tax on your pension.
